Dear Valued Patient,

Welcome to Snow Family Medicine! We are honored that you have chosen our practice for your healthcare needs. Our mission is to provide compassionate, comprehensive, and evidence-based care for the entire family in a respectful and patient-centered environment.

To help ensure a smooth and positive experience, we have outlined below our office policies and expectations. Please take a few moments to review this information.

📅 Office Hours & Scheduling

Regular Hours: Monday–Thursday, 8:00 AM–5:00 PM. Friday appointments available by request until 2 PM.

Appointments: Please call or use the patient portal to schedule visits. We kindly ask that you arrive on time; patients arriving more than 10 minutes late may need to be rescheduled.

Cancellations: A minimum of 24 hours’ notice is required. Repeated no-shows or late cancellations may result in limited scheduling privileges.

📞 Communication & After-Hours

Phone Calls & Messages: We return all voicemails within 1 business day. You may also reach us through the secure patient portal for non-urgent matters.

After-Hours Care: For urgent concerns outside business hours, you may press “4” when calling our main line to reach the on-call provider. Non-urgent requests, including refills and forms, will be addressed during normal office hours.

🔐 Medical Records & Privacy

Records Requests: Must be made in writing and include a signed release form. Please allow up to 7 business days for processing.

Confidentiality: We follow all HIPAA regulations to protect your health information.

💳 Insurance, Billing & Payment

Insurance: We participate with most major insurance plans. It is your responsibility to verify coverage and copay details.

Payment: Copays and outstanding balances are due at the time of service. Self-pay patients are eligible for same-day discounted rates.

💊 Prescriptions & Refills

Routine Refills: Please allow 48 hours for processing. Refills will not be completed after hours unless for life-sustaining medications.

Controlled Substances: Require regular follow-up visits and cannot be refilled early.

📄 Forms & Prior Authorizations

Disability, Work & School Forms: May require up to 5 business days for completion. Some forms may incur a processing fee.

Prior Authorizations: Handled by our staff upon provider approval. We will keep you informed throughout the process.

🧼 Office Environment

We maintain a clean and safe environment. Please supervise children and be respectful of staff and fellow patients.

Any threatening, aggressive, or inappropriate behavior will not be tolerated and may result in dismissal from the practice.

No-Shows, Missed Appointments and Late Check-in Office Policy

 o   Please be advised that patients arriving 15 minutes late for their appointment will be considered a no-show and will need to be rescheduled.

 o   After 2 no-show occurrences, the practice may elect to discontinue your care with our providers, and you will need to seek care elsewhere.

o   NOTE: You may receive a bill for visits that you do not arrive to and do not call ahead of time to cancel.

We are very understanding of life’s unpredictability and will try to work with and accommodate you in the best way possible when rescheduling. We ask that you please be courteous of those who may need a visit with us urgently.

Financial Policies & Responsibilities

To streamline billing and ensure timely payment, Snow Family Medicine, LLC requires a valid credit or debit card to be securely stored on file at the time of service.

We utilize Square, a trusted and HIPAA-compliant payment processor, to securely encrypt and store your payment information. Your card will only be charged for:

Any copayments, coinsurance, or deductibles not collected at the time of visit

Balances remaining after insurance processing

Missed appointment or late cancellation fees, if applicable

You will receive a notice before any charge is processed. By receiving care at Snow Family Medicine, you agree to this policy.

Payment Processing and Returned Check Policy

Credit/Debit Card Fees: Patients who choose to pay by credit or debit card will be responsible for any associated processing fees, typically estimated at 4%.

Returned Checks: A $35.00 returned check fee will be assessed for any check returned due to insufficient funds or other banking issues. This fee is in addition to any charges assessed by your bank.

All fees must be paid in full before future services can be rendered. 

You will receive a notice before any charge is processed. By receiving care at Snow Family Medicine, you agree to this policy.

Delinquent Account and Dismissal Policy

Patients with outstanding balances exceeding 120 days and no prior financial arrangements made with our billing department will be subject to termination of care and formal dismissal from Snow Family Medicine, LLC.

To avoid disruption in care, patients are expected to promptly address all balances or contact the billing department to establish a payment plan.

Non-compliance may result in written notice of dismissal and transfer of care responsibilities.
